Before your first appointment, please print and fill out the following three forms (there are 4 pages in total) and bring them to your first appointment.
When you arrive at the office you will see a brown light switch on the left hand wall. Please lift up the switch and leave it up. The light switch will signal your arrival and Dr. Snider will be with you shortly.
If you would like to use your insurance you will need to contact your insurance company to determine what they pay for a Clinical Psychologist who is a Non-Preferred Provider. Dr. Snider generates an insurance form with all the necessary information and gives the form to you the first week of the new month with all of the previous months charges. Payment is due at the time of services or if you prefer you can pay once a month when you receive your monthly statement.
